Getting a Disability Certificate (UDID)

What it is
A disability certificate says you have a disability.
UDID is a card with a number. It works all over India.
You need it for schemes, concessions, jobs and exams.
What you need
A photo.
Aadhaar or another ID.
Any old medical papers.
Where to go
Apply on the UDID website, or ask the District Hospital.
A doctor will check you.
Someone you trust can come with you.

“Please give me this paper in Easy Read.”
Details
UDID (Unique Disability ID) is issued under the RPwD Act 2016 through the Department of Empowerment of Persons with Disabilities. Applications are made online on the UDID portal or through the district medical authority; the certificate is issued after assessment by a medical board. Keep the application number.
